Our purpose
We empower people with kidney disease to live each day to the fullest.
Our core focus is on helping people living with kidney disease—first and foremost. These patients may have kidney disease, but it doesn’t define them. We’re here to provide the right resources and support to help them do better each day. Our hope is that patients spend less time worrying about managing their health and more time doing what they love.
Our Interdisciplinary Care Team
Though not every role is live in all markets, here is our team approach to whole-person care and the wraparound services our IDT provides.
- Nurse Care Manager – Helps patients with are plan, educates, and coordinates medication care services.
- Social Worker – Offers caregiver guidance and emotional support.
- Care Coordinator – Helps connect patients to resources, sets up their care, keeps team up to date, and makes sure needs are met.
- Dietitian – Offers nutrition and dietary support to help patients feel their best.
- Nephrologist — Leads care team and treatment plan.
